In all my trips I've never paid extra for 1st class and have never been uncomfortable. IMO, train seating on the intercity trains are a hundred times more comfortable than your average plane seat. (Only slightly exaggerating). On German trains, reservations aren't usually required on the intercity trains .The only real difference is the seating comfort, see the photos on this page, but in first class food & drink orders are taken by a steward at your seat so you don't have to go to the restaurant car yourself, a useful service.All trains in Germany have toilets, and most intercity trains come with bistro and restaurant cars serving hot meals, snacks and beverages. There's no issue with bringing your own food and drink.

Do German regional trains have 1st class : With Flexpreis and Flexpreis Plus, 1st class includes a reserved seat. This applies to both long-distance and regional services (on selected trains).
